Osteoporosis and Frailty: What 2026 Research Reveals About Supplements

VITAMIN D AND FRAILTY: WHY STEADY BEATS MEGA-DOSES

Researchers Viola and colleagues examined how vitamin D works across many body systems, contributing to muscle strength, bone renewal, immune health, and energy production. (1) Low vitamin D levels are linked to muscle loss, weaker bones, and the onset of frailty. However, supplements seem to offer the most benefit when someone is genuinely deficient or especially vulnerable, and big, infrequent "mega-doses" have been linked with more falls and fractures. Bottom line: know your numbers, and choose steady, recommended doses over occasional large ones.

CALCIUM AND VITAMIN D: A FOUNDATION FOR OSTEOPOROSIS CARE

A review by Pascual-Pastor and Barceló-Bru named daily calcium and vitamin D an essential foundation of osteoporosis care. (2) The authors summarized the science on how the two nutrients work together to reduce fracture risk and protect bone density, and noted that vitamin D deficiency is still widespread even in people who already have osteoporosis. They also highlighted the importance of supplementation for those using osteoporosis medications.

DO PROBIOTICS HELP BONE DENSITY?

Probiotics told a different story. In a well-designed trial, Resciniti and colleagues gave 114 women in the first years after menopause either a daily probiotic or a placebo for a year. (3) The probiotic did not slow bone loss, a valuable reminder that promising theories still need proof, and that no single capsule replaces a thoughtful plan.
