Chronic low back pain like that explained by the veteran in this case report is very common and can be quite disabling. Spinal manipulation is recommended for the management of chronic low back in several guidelines backed by such groups as the American Pain Society and the American College of Pain Physicians. (2,3) With chronic low back pain, central sensitization – the effect of repeated nerve stimulation has on the central nervous system - is purported to contribute to chronicity of pain.  Spinal manipulation’s benefit may well come from its influence on this sensitization. A systematic review and meta-analysis of randomized controlled trials (47 RCTs and 9211 participants) of spinal manipulation care for chronic low back pain described that spinal manipulation produced similar effects to other suggested therapies of chronic low back pain (like exercise and drug therapy), was superior to non-recommended intervention for short-term improvement, and had limited, mild-to-moderate, fleeting adverse effects. (5) Another such meta-analysis determined that spinal manipulation appeared to be a good option for relief of pain and improvement of function for chronic low back pain sufferers. (6) In a meta-analysis of specifically older (ages 65-91 y/o) chronic low back pain sufferers, the same deductions were reported: spinal manipulation should be considered a beneficial treatment for the older patient with chronic low back pain for pain relief and function improvement. (7) Most spinal manipulation is rendered by chiropractors according to the National Center for Complementary and Integrative Health.
